Membership Terms
Membership is non-transferable. Membership discount cannot be applied with any spa gift certificates. You may cancel your Spa Membership after the initial 6-month contract without penalty by providing written notice of your request at least 30 days prior to your billing date. If you cancel before your contract ends, a cancellation fee of one month will be issued. Benefits must be used during membership month, otherwise, they are forfeited. The 10% Off All Retail Products is valid up to 6 products in a month.
Auto-renews Terms
Your Spa Membership Includes 6-month initial contract, month to month thereafter. Members wishing to have a Customized Peel, Skin Rehab, Bright Skin Facial must pay $20 in addition to their monthly membership fee. Services do not roll over nor can be transferred, if they are not used within the agreed billing cycle they will be forfeited.
TO CANCEL YOUR SPA MEMBERSHIP After your three-month Spa Membership is completed, you may cancel by providing written notice of this request at least 30 days prior to your last billing date, otherwise the continued month-to-month membership will remain active. If you wish to cancel your membership during your three-month contract, we require 30 days written notice and charge an early cancellation fee, which will be billed to your account. E-Mail the written notice to: [email protected]. Buyer hereby authorizes Ayurvida Wellness and Beauty to make monthly charges / withdrawals from (a) the account used to pay the initial payment described above, or (b) a replacement account hereafter used for payment of any sums due to Ayurvida Wellness and Beauty under this Spa Membership Agreement. Spa Membership fees will continue each month following the initial payment date described in this Spa Membership Agreement. In the event that sufficient funds are not available in Buyer's account or debits are otherwise not accepted, Ayurvida Wellness and Clinic may resubmit the charge. If the Spa Membership fee is returned uncollected for a second time, the buyer’s membership will be placed on hold until the monthly fee is remitted in full. During this time, no unpaid services may be redeemed. If balance due is not remitted in full within 30 days from the due date, this contract will become null and void and the buyer’s membership agreement will be discontinued. Conditions Buyer understands that Ayurvida Wellness and Beauty receives written notice of termination of this Spa Membership Agreement and has a reasonable opportunity to act on that notice. If you decide to change your billing information, a 10-day notice is required. Buyer may timely notify the financial institution in control of Buyer's account to terminate this EFT/CC Request, but such notification will not otherwise affect this Spa Membership Agreement and Buyer's obligations herein. By signing this Spa Membership Agreement, Buyer acknowledges that Buyer is of legal age; has received a filled-in and completed copy of this Spa Membership Agreement that identifies the Spa Membership type and services purchased and has read and understands the entire Spa Membership Agreement that includes but not limited to the *EFT/CC Request. This Spa Membership Agreement constitutes the entire agreement of the parties, and no other agreement or understanding exists between Buyer and Ayurvida Wellness and Beauty. Has not expressed or implied any warranty or representation other than those set forth in this Spa Membership Agreement to induce Buyer to enter into this Spa Membership Agreement.
Ayurveda + Christianne have absolutely changed my life.
